Hospice Care



Criteria for Home Health Care under Medicare:

Patient must be Home-bound and unable to function outside of the home without a considerable and taxing effort (i.e. with the aid of another person and/or device).

Have an exacerbation of an existing illness of onset of a new disease process

Respite care to relieve the family/caregiver for up to five (5) days per stay per period.


Criteria for Hospice Home Care under Medicare.

Intermittent visits by nursing personnel, a homemaker, home health aide, social worker, chaplain, volunteers and other professionals as defined on the Informed Consent Form.

Intermittent visits and/or consultation by auxiliary team members providing

SN, PT, or SLP need on an intermittent basis
